About Us
The healthcare system is experiencing dramatic reform through major system redesign. Mercy Care is well positioned to contribute significantly to an integrated care system, as healthcare transitions to a much broader community health goal.
Mercy Care’s social model of informal elder support services provides opportunities for greater stability and continuity in older adults’ care. Availability of community support services have long-term health consequences and are part of the new health care model.
Mercy Care recognizes that according to the New York State Office for the Aging, by 2040 in the Adirondack North Country region, which includes the counties of Clinton, Essex, Franklin, Hamilton, Jefferson, Lewis, and St. Lawrence, the only population growth projected is in the category of people over the age of 60. The need for Mercy Care’s services will grow.
The aftermath of the coronavirus pandemic is causing an epidemic of loneliness and social isolation among adults, carrying with it significant health and emotional risks, according to a report from AARP Foundation and the United Health Foundation.
